Gold Card:
The Gold Card is a Texas public health benefit provided by the Harris County Hospital District that provides financial assistance for medical care for Harris County residents. Based on your household income, you may qualify for partial financial assistance, on a sliding scale, including: full pay, half pay or nominal co-pay. VN TeamWork is currently offering free application assistance
Provide:
· Texas I.D. or drivers license stating the person lives in Harris County
· Social Security Card
· Citizenship(Passport or Birth Certificate) or Green Card
· Proof of income (1 months check stubs)
· Any bills or bank statement (within 60 days)
· Income tax (if necessary)
· Medicaid or CHIP card (if necessary)
Application assistance available Monday through Friday from 9am-12pm
